Custom forms

Your company — or a client you work for — can build additional report forms tailored to specific work, for example a Daily Site Audit, a Lost Property form, a Suspicious Activity form, or a Visitor Sign-in Sheet. These are created on the management side using a Form Builder tool and published for guards to use. A client can have more than one form of their own — each appears as its own button.

Where to find custom forms

Open the Reports tab and tap New Report to see the type picker. What you see depends on whether you are clocked in:

  • Clocked in to a venue — if that client has their own custom forms, they are pinned at the top of the list, under a heading with the client’s name, above the standard report types. Below them is a Standard reports section (Incident, Patrol, Use of Force, Shift, Vehicle Defect, Vehicle Check). Some clients choose to hide the standard types on their own venues — if so, you can still reach them via See More (they are never taken away from you, just tucked out of the way).
  • Not clocked in, or filing for a past shift — tap See More at the bottom of the picker to expand a list of custom forms from other clients you have worked at, grouped by client, with a search box to find one quickly. This is how you file a report for a shift that has already ended, or fill in a form while off-shift.

Every custom form card shows the client’s name at the top, so you always know which client it belongs to. Your company’s own firm-wide custom forms always appear in the Standard reports section, on every site.

Forms may also be linked directly from the Quick Actions row on your Dashboard if your manager has set that up.

How to complete a custom form

  1. Open the Reports tab and tap New Report, or tap the relevant Quick Action on the Dashboard.
  2. Select the custom form from the type picker.
  3. Complete the fields. Each form is different — some may include text boxes, dropdowns, photo uploads, or signature fields, depending on how the form was built.
  4. Tap Submit when done. If you cannot finish in one go, the form saves as a draft for you to return to.

The lifecycle

Custom forms follow the same lifecycle as standard reports:

  • Submitted — Received and waiting for a manager to review.
  • Approved — The manager has accepted it and it is filed.
  • Amendments requested — Your manager has asked you to add or correct something. You will receive a notification.

If a form is mandatory

Your manager will tell you if a particular form is required for a task or at a specific point in your shift (for example, a vehicle check at the start of every shift that involves fleet vehicles). If it is mandatory, complete it before starting the associated task.

If you cannot find a form

Custom forms may only be available on certain sites, to certain roles, or for certain shift types. If your manager has told you to file a specific form and it does not appear in your picker, check with them — it may need to be enabled for your account, or may be accessible only on the relevant site.

Tips

  • Read the entire form before filling it in — some questions are conditional (later fields may only apply depending on your answer to an earlier one).
  • If a field asks for a photo, attach one rather than writing a description. A photo is faster, clearer, and adds evidential weight.
  • Custom forms are still official records. Apply the same care and accuracy you would to any other report.
